Changing your name with the CSP

Use this form to tell us if you have changed your name - for example as a result of marriage, divorce or by Deed Poll ("Recording Change of Name" in Scotland).

Please note you will need to attach an electronic copy of documentation proving the change. This can be either a copy of your marriage or Deed Poll certificate. A scan or photograph will suffice.
